Story Behind the Curve

  • Prediabetes, a precursor to diabetes, occurs when blood sugar is higher than normal, but not high enough to be diagnosed as diabetes. There may be no external symptoms of disease.
  • Older adults, African Americans, and American Indians are at higher risk for prediabetes. The lower the annual household income, the higher the prediabetes prevalence, and while differences by income are not statistically significant, the pattern suggests an association with income.
  • In the third quarter, the Diabetes Prevention and Control Program (DPCP) worked with the American Association of Diabetes Educators and HealthInsight New Mexico (HINM) to coordinate efforts to increase access, awareness, and infrastructure to support the National Diabetes Prevention Program (NDPP). Both organizations received federal funding for diabetes prevention in New Mexico.
  • DPCP’s NDPP Master Trainer Select provided a refresher course for nine Lifestyle Coaches; technical assistance and coaching to delivery sites on the recognition application process and centralized referral and data system (CRDS); and training and orientation to UNM Hospital on the CRDS. The referral system contractor provided a training to HINM on the CRDS. The DPCP submitted articles on the Paths to Health initiative and information about the CRDS in the New Mexico Medical Society’s (NMMS) newsletter, Medical News
  • In the 4th quarter, the DPCP will convene diabetes prevention stakeholder meetings to finalize New Mexico’s diabetes prevention action plan and a New Mexico Paths to Health Collaborative meeting. One Lifestyle Coach training will be offered. Information about NDPP will be shared with NMMS’s Liaison Committee to the Human Services Department.

What Works

  • NDPP, a one-year lifestyle balance curriculum developed by CDC for people with prediabetes, is based on the original DPP study that demonstrated that 5-7% weight loss achieved and maintained through regular, moderate physical activity and improved nutrition, prevented or delayed the progression of prediabetes to diabetes by 58% compared to standard lifestyle recommendations. The program is most successful in older adults, who are also at greater risk for developing diabetes than younger adults.

Strategy

  • Raise awareness about prediabetes and NDPP among providers.
  • Increase availability (Build program sites, train lifestyle coaches).
  • Work with health systems and community organizations to increase screening, testing, and referral using a DPCP referral system.
  • Work with health plans and large employers to establish health plan coverage and increase access to NDPP.
