Story Behind the Curve

The Strong Families Vermont Nurse Home Visiting Program is an evidence based home visiting model. The data is based on Strong Families Vermont collection protocols which are collected by nurse home visitors and entered into the Strong Families Vermont database. The data indicate that 79.5% of children enrolled in Strong Families Vermont have been screened for developmental delay by 9 months, indicating fidelity to the model guidelines. Vermont's Nurse Home Visiting Program has been going through some changing recently which would account for 2018 Q4 data to have dipped so drastically. The children were unable to locate in time to perform the screening during the designated time period. Efforts to find additional methods to contact families and children are being explored by each local program.

At this time, there has been some difficulty with clients being engaged at the time of ASQ screening. MCH will continue to monitor Strong Families Vermont implementation via Vermont's home health agencies until it is determined that there is ongoing maintenance of screening all children in the Strong Families Vermont Nurse Home Visting Program for developmental delays by 9 months with the validated tool. All local agencies are currently engaging in practices to increase frequency and type of follow-up activities with families.

Partners

Vermont's Home Health Agencies are the host of the Strong Families Vermont Nurse Home Visiting Program. The Strong Families VT program is a service provided under the Children Integrated Services (CIS) umbrella. Community partners include: CIS teams members, CIS coordinators, Local Health District office staff, Community Advisory Board, Child Development Division (CDD), Primary care physicians, OB/GYN providers, School nurses, and Planned Parenthood.

What Works

Maternal and child health nurse home visitors are the backbone of Strong Families Vermont Nurse Home Visiting Program. A nurse-led maternal health and home visitation program partners low-income, first-time mothers with maternal and child health nurses. The resulting trusted relationship gives the mothers the support they need to have a healthy pregnancy, improve their child's health and development, and become more economically self-sufficient.
