- Onsite visits to 2 SNFs with lowest resident vaccination rates
- Arranged vaccination clinics in SNFs to vaccinate residents
- Arranged for our mobile vax team to vaccinate single SNF residents
Update 12/8/21
Connected with all the the SNFs that are less than 85% and offered evidence-based best practices on how to boost their vaccination rates. The SNF with the lowest resident vax rates improved from 68% of residents vaccinated to 86% thanks to a vaccine clinic that we arranged for them on 11/17/21.
