The Surry County Health and Nutrition Center's Diabetes Educators have been working with patients on Diabetes Self-Management for many years now.  They were still able to work with patients despite COVID-19 by virtual and phone appointments but the number of patients seen did decrease once in-person visits were no longer an option.  We are slowly re-opening in-person visits at this time.

The Health Department works with many doctor's offices and other medical centers by getting referrals for new patients.

 The personalized, comprehensive education provided in both individual and group sessions has been helpful for patients and having them take the initiative to manage the disease while the Diabetes Educator monitors the progress has been very effective for most.

The health department will continue offering this service.
