What Works
Clients receive nutritional education to include: food labels, portion sizes, counting carbs, My Plate, etc.
Clients receive physical activity education to include: benefits of physical activity, increasing physical activity, types of physical activity, etc.
Clients set SMART (Specific, Measurable, Achievable, Realistic, Timely) goals for effective diabetes management.
Action Plan
Continue providing DSMES program to clients with education on nutrition, physical activity, and monitoring blood sugar.
Assist clients with setting SMART goals.
