Story Behind the Curve
The purpose of the DMH Housing Subsidy Plus Care Program aims to provide permanent supportive housing to individuals with mental health conditions who are experiencing homelessness or are at risk of homelessness. The program is specifically designed for people who are in hospital or acute care settings and need to exit these facilities, or those who require support to leave hospital beds but have no available housing options.
Adults who are eligible for the DMH Housing Subsidy Program are experiencing homelessness or are at risk of homelessness, have a disabling mental health condition and/or are either in hospital or acute care settings or require help transitioning out of hospital beds but have no housing available.
The number of individuals served has remained steady, but significant rent increases in communities have put constrains on how many clients the program is able to serve, with potential decline in the future.
