Story Behind the Curve
These results are based upon the staff's weekly rating of each resident's daily living skills averaged across the quarter. The scale (attached below) to assess levels of independence across the domains of meal planning and preparation, medication management, chores, social skills, symptom management, personal hygeine, co-occuring disorder and substance use, and physical wellness, using the following rating scale:
1 – Unable to do at all, staff completes
2 – Requires daily reminders, staff assists when needed
3 – Requires staff reminders/assistance 1-4 times a week
4 – Requires staff reminders/assistance 1-4 times per month
5 – Able to complete independently
The percentage above represents the % of residents who score 4 or higher on this rating scale.

1/19/16 - Staff meeting with Program Manager and Outcomes Staff
One staff reports that she feels that My Pad is more of a community. The staff look forward to seeing residents, and the residents look forward to seeing the staff. Both plan activities together and hang out together. There is a genuine community-feel balanced with personal homes for privacy.
The staff developed the charting system listed above and the group discuss residents using the system. The program manager likes to use the system because he believes that it helps the staff to check on areas of a person's life that they might otherwise forget to check thus ensuring that all areas of a person's life are considered when trying to maximize their independent living skills (broadly defined).
