People who have a PCP are less likely to use the Emergency Department for routine care. Of the people who report not having a primary care provider (PCP), the agency can demonstrate that it offers help services or referrals to obtain a PCP.
0 – Criteria not met (Agency has no process or plan to offer assistance in obtaining a PCP).
1 – Criteria partially met (Agency may suggest or recommend that the client obtain a PCP but has no documented process or plan in place, and/or no data can be queried via the Electronic Health Record).
2 – Criteria met (Agency has documentation in the form of a process or policy that directs staff to provide people without a PCP assistance with securing a PCP AND this information can be queried). Agency supplies the result of the calculation above to be used to calculate future benchmarks.
