Story Behind the Curve
It is important to provide regular follow-up treatment to people after they have been hospitalized for mental illness. Research recommends that a clinical or support service with agency staff soon after discharge reduces the likelihood of readmission to the hospital and improves the transition back to the community. Follow up care also helps health care providers detect early post-hospitalization reactions or medication problems, and provide continuing care. Note: This measure is for CRT only so that it is aligned with the national HEDIS measure set.
