This scorecard holds the FY17 semi-annual and annual performance measures for reporting purposes for the Pathway to Services/LAM-Family Navigation Program in Montgomery County.
The Family Navigation component will include empowering the family by modeling, mentoring, educating, coaching and linking them to other families to train them in their ability to advocate for their child. The Family Navigators will: provide emotional support to client families; provide information on various services and mandates within the system of care; link families with assessment/evaluation and formal and informal direct service resources; and, empower them to ultimately act as their own best advocates.
