Regional Chronic Disease Scorecard and Toolkit

This Chronic Disease Prevention & Management Regional Priority Scorecard was created to support WNC hospital and health departments address this regional priority at a local level by:

• Connecting to regional partners, evidence-based interventions, local news & research and funding opportunities for Chronic Disease
• Displaying shared Programs/Strategies/Processes across the region for peer support, alignment, and learning
• Identifying shared Performance Measures & regional comparison data
• Encouraging collaboration

This scorecard/toolkit was co-developed and prototyped by members of the WNC Healthy Impact Regional Priority Ad Hoc Workgroup, including hospital, health department, and key regional partners.
