Regional Social Determinants of Health Scorecard and Toolkit

This Social Determinants Regional Priority Scorecard was created to support WNC hospital and health departments address this regional priority at a local level by:

• Connecting to regional partners, evidence-based interventions, local news & research and funding opportunities for Social Determinants of Health
• Displaying shared topics of interest across the region for peer support, alignment, and learning
• Identifying shared Performance Measures & regional comparison data
• Encouraging collaboration

This scorecard/toolkit was co-developed and prototyped by members of the WNC Healthy Impact Regional Priority Ad Hoc Workgroup, including hospital, health department, and key regional partners.

The following WNC counties listed Social Determinants of Health-related priorities in their 2018 Community Health (Needs) Assessments:

Cherokee – Access to Healthcare

Henderson – Safe & Affordable Housing

Mitchell – Access to Healthcare/Social Determinants of Health

Yancey – Social Determinants
