The HNC 2030 Scorecard for Lee County is used to track progress and provide a narrative for the Community Health Improvement Plans generated from the 2022 Community Health Assessment.
Our current priorities for the Community Health Improvement Plans include healthy eating, active living, mental health, substance use and abuse, social determinants of health, and teen pregnancy. Results are sought after through multiple programs and activities, which are described below.
The pandemic and effects of COVID-19 have interfered with the County's ability to implement new and existing programming since 2019. There are gaps that exist from 2019 to 2022 that are due to COVID-19 restrictions, precautions, and barriers.
Partners directly involved or responsible for implementation include Lee County Health Department, Lee County Schools, Lee County Government, NC Cooperative Extension, Partnership for Children and Families, Sandhills Center, Lee County CIT, Local Law Enforcement, first responders, and others.
