HNC 2030 Scorecard: Iredell County 2019

The following Clear Impact Scorecard was created and submitted August 12, 2022 in order to meet the requirements for the Iredell County State of the County Health Report (SOTCH) and the Community Health Improvement Plan (CHIP).

Clear Impact Scorecard™ is a management AND performance management tool that allows organizations to collect, analyze, and report specific data. The powerful accountability tool measures the impact of an organization and improves the performance of programs while reporting progress of goals and/or missions to funders, stakeholders, partners, and the public. The tool supports partnerships by allowing organizations to collaborate and contribute data to achieve a common goal. Scorecard assists communities organize their community health improvement efforts through a web-based browser. The Iredell Health Department is using Clear Impact Scorecard™ to support the development of the electronic CHIP and SOTCH Report in communities in Iredell County.

The Iredell County Health Department is excited to share the Healthy NC 2030 Scorecard for Iredell County. This Community Health Improvement Scorecard is an easy way to learn about some of the efforts currently underway in Iredell County to address three health priorities identified in the 2019 Iredell County Community Health Assessment (CHA): 

• Infant Mortality (Disparity ratio amongst White and African Americans)
• Mental Health   (Suicide between the ages of 11 and17)
• Chronic Disease (Youth Vaping and Municipality Tobacco Policy)

Iredell County and our community partners are united in our efforts to support community health improvements to address these priorities.
