HNC 2030 Scorecard: Davidson County 2021-2024

The 2021 Community Health Needs Assessment (CHNA) identified the following priority areas: 
-    Substance Misuse
-    Chronic Disease
-    Mental Health
The following Community Health Improvement Plans (CHIPs) were created and will be submitted to the North Carolina Department of Health and Human Services (NCDHHS) in the fall of 2022. The CHIPs were developed utilizing the “Results-Based Accountability” (RBA) framework to address the top health priorities identified in the CHNA, in both the long- and short-term. The CHIPs are published using Clear Impact Scorecard (Scorecard) and include desired results, indicators, strategies, and performance measures, and will be routinely updated to reflect the efforts in place across Davidson County.
