The health priorities identified in the 2017 Daive County Community Health Assessment (CHA) are:
- Chronic Disease Management
- Drug Misuse and Abuse Prevention and Education
- Access to Affordable Care
A Community Health Assessment is conducted by Davie County Health and Human Services, Division of Public Health, every three years. Due to COVID-19, an extension was granted, and the next Community Health Assessment (CHA) will be published in early 2023. In the years in between each CHA, a State of the County Health (SOTCH) report is published to act as a brief update in between the larger reports.
The HNC 2030 Scorcecard for Davie County will act as a digital record for results, indicators, programs, and performace measures. This scorecard will aid in accountability and keep Davie County intentional in their work.
This Community Health Improvement Plan (CHIP) Scorecard is designed using Results Based Accountability (RBA). Results Based Accountability (RBA) is a disciplined way of thinking and taking action that is used to improve the quality of life in any given population. RBA starts with the ends and works backward to the means. For example, in targeting chronic disease, our 'ends' or Result is "All Davie County residents have long and healthy lives." We then work backwards, looking at the programs that bring us to this result, measuring the progress of the programs, and monitoring the data that serve as indicators for our success.
Our Community Health Improvement Plan (CHIP) includes the 'ends' or desired results for each priority area, indicators for monitoring progress on our results, the programs leading to the desired results, and performance measures for the programs. Our CHIP is a long-term plan which guides our work, three years at a time.
Common Language Key
R - Result
A population result (or outcome or goal) is a condition of well-being for children, adults, families, and communities in plain language.
I - Indicator
A population indicator or benchmark is a measure that helps quantify the achievement of a result. Indicators answer the question, “How would we recognize this result if we fell over it?”
P - Program/Strategy
A strategy is a coherent set of actions that has a reasoned chance of improving results. Strategies are made up of our best thinking of what works, including policy, systems, and environmental changes and programs. Strategies operate at both the population and performance levels.
PM - Performance Measure
A program or service performance measure or indicator is a measure of how well a program, agency, or service system is working.
