HNC 2030 Scorecard: Duplin County

The Duplin County Health Department is excited to share the Healthy NC 2030 Scorecard for Duplin County. This Community Improvement Scorecard is an easy way to learn about some of the efforts currently underway in Duplin County to address the three health priorities identified in the 2021 Duplin County Comunity Health Assessment (CHA):

1. Access to Care
2. Chronic Disease 
3. Mental Health 

While our community has been adversely impacted by the COVID-19 pandemic since March 2020, Duplin County Health Department and our community partners are united in our efforts to support community health improvements to address these priorities. This Scorecard also serves as Duplin County's Community Health Improvment Plans (CHIPs), fulfilling the NC Local Health Department Accrditation reqiremetns that local health departments submit two CHIPs following the CHA submission. 

For each priority, the Scorecard highlights:

• A Results Statement, a picture of where we would like to be, 
• Important local indicators or measures of how we are doing linked to Healthy NC2030 indictors and
• Select Programs or activities and
• Key Performance Measures that show how these programs are making an impact. 

The Scorecard also contains the annual Duplin County State of the County Health reports or SOTCH.
