The following CHIP Scorecard was created and submitted September 1st in order to meet the requirements for the Anson County Community Health Improvement Plans.

Clear Impact Scorecard™  is a web based tool that collects, tracks, and analyzes data to measure the impact of a specified goal while improving the performance of programs.

Anson County is using Clear Impact Scorecard™ to support the development of electronic CHIPs and SOTCH Reports.

Scorecard assists communities in sharing a common vision to improve health and disparity issues in their community.
