HNC 2030 Scorecard: Lincoln County (2022-2025)

The Lincoln County Health Department is excited to share the Healthy NC 2030 Scorecard for Lincoln County. This scorecard will track progress on the community health improvement plans resulting from the 2022 community health assessment (CHA). During the 2022 community health assessment, the CHA team identified three health priorities to focus on at the local level:

• access to care
• health behaviors
• substance use

In addition to the community health assessment and community health improvement plans, this scorecard will include Lincoln County’s annual State of the County Health (SOTCH) reports for the current CHA cycle.
