ABOUT THE REPORT
The 2021-2023 Hyde County Community Health Improvement Plan (CHIP) provides a detailed description of specific community health goals, as well as the means by which these goals are intended to be achieved. Each goal was designed to address the health needs of the Hyde County community, as identified in the 2021 Community Health Assessment (CHA). These priority health issues include: 1) Mental Health, 2) Substance Use, and 3) Child Abuse/Neglect.
COMMUNITY PARTNERS
The CHA process, from data collection to action planning, brings together a vast array of community partners with the ultimate goal of improving the health and quality of life of Hyde County. These community partners work together to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measurable health improvement, which is outlined in the form of a CHIP. We would like to thank each and every one of our community partners for aligning their efforts and supporting one another for the greater collective impact on the health of Hyde County.
