Welcome to the 2022-2026 Community Health Improvement Plan (CHIP)
The Brunswick County Community Needs Health Assessment (CHNA) is conducted at least every four years to determine the community’s needs and develop programs and plans to address the health priorities. The final priority areas identified from the 2022 CHNA and used in the planning and creation of the CHIP are:
- Chronic Conditions
- Behavioral Health
- Access to Healthcare

Results-Based Accountability
Results-Based Accountability (RBA) is a data-driven, disciplined way of thinking and acting to improve complex health issues. RBA drives this community health improvement plan. In the table below, the key components of RBA have been defined.
