The 2022 Community Health Assessment priority areas are: 

  • Mental Health
  • Substance Use Disorder 
  • Obesity With Risk Factors 

The following Community Health Improvement Plan (CHIP) Scorecard was created and submitted by September 1st, 2023, to meet the Burke County Community Health Improvement Plan requirements.

A Key to Navigating this Scorecard:

 Community Health Assessment 

 Results - Guiding Objectives that are desired to be reached in the CHIP efforts. 

 Indicator - The data set that helps justify the need for the program in reaching the end result. 

 Programs - Programs that are implemented or are existing that are used to address the Community Health Assessment Priorities. 

 Performance Measures - Measures to indicate how well the program is working or to indicate areas on improvement. 

 SOTCH Report- State of The County Health Report used to update yearly on the progress of the programs implemented in the Community Health Improvement Plan

 

  • Implementation of all programs with the Community Health Improvement Plan will be implemented by Burke County Public Health and members of the Burke Wellness Initiative through the promotion of the programs.
