Scotland County's Community Health Needs Assessment is conducted in collaboration with Scotland Health Care System every three years to determine the community's needs and develop programs and plans to address the determined health priorities.
Afterwards, a Community Health Improvement Plan (CHIP) is created and includes how the health priorities will be addressed and how progress will be measured. The CHIP will help guide the work of Scotland County Health Department, Scotland Healthcare System and our community partners for the priorities selected.
The 2022 Scotland County Community Health Assessment health priorities are as follows:
- Obesity/Overweight (Hypertension, Heart Disease, Diabetes and Cancer)
- Mental Health and Substance Use/Misuse including Tobacco/E-Cigarettes
- Positive Youth Engagement and Healthy Behaviors
