The HNC 2030 Scorecard for Davie County will act as a digital record for results, indicators, programs, and performance measures. This scorecard will aid in accountability and keep Davie County intentional in their work.
This Community Health Improvement Plan (CHIP) Scorecard is designed using Results Based Accountability (RBA). Results Based Accountability (RBA) is a disciplined way of thinking and taking action that is used to improve the quality of life in any given population. RBA starts with the ends and works backward to the means. For example, in targeting chronic disease, our 'ends' or Result is "All Davie County residents have long and healthy lives." We then work backwards, looking at the programs that bring us to this result, measuring the progress of the programs, and monitoring the data that serve as indicators for our success.
Our Community Health Improvement Plan (CHIP) includes the 'ends' or desired results for each priority area, indicators for monitoring progress on our results, the programs leading to the desired results, and performance measures for the programs. Our CHIP is a long-term plan which guides our work, three years at a time.
