Horizon Public Health Community Health Improvement Plan 2023-2027

This Community Health Improvement Plan (CHIP) guides our community in the development of initiatives, strategies, and policies aimed at addressing the top health priorities identified in our community. Horizon Public Health and its partners are committed to working to implement, monitor, and document the strategies outlined in this Community Health Improvement Plan. The Community Partner Leadership team used the Mobilizing for Action through Planning and Partnerships framework to complete a Community Health Assessment which assisted in identifying the following top health priority areas for 2023-2027:

The Horizon Community Partner Leadership Team will continue to work to improve and maintain the capacity and infrastructure present in the Horizon Public Health service area. This plan will be used as a guide to work collectively in engaging key stakeholders and community members to take ownership, participate in, and document strategies identified to meet the objectives outlined in this plan.

Access to Care Goal: All have access to care in the Horizon Public Health service area.

  1. All people have access to preventative dental screenings.
  2. All community members can access mental health resources.
  3. All families can access and utilize child care when needed.

Community Resilience Goal: All community members experience positive mental well-being and live free from the harms of substance misuse and abuse.

  1. Adults and youth have positive mental well-being.
  2. All community members are free of substance misuse and abuse.

Chronic Disease Goal: All community members have the opportunity to achieve optimal health.

  1. All community members have access to physical activity opportunities and healthy foods options.
  2. Cross-organization communication and collaboration to better serve communities.

Horizon Public Health extends gratitude to the partners who participated in the development of the CHIP. This plan represents the commitments of partners to improve the health well-being of our community.
