HNC 2030 Scorecard: Iredell County 2023
The Clear Impact Scorecard presented here was precisely crafted and submitted on March 1, 2024, in fulfillment of the prerequisites outlined by the Iredell County Community Health Assessment (CHA), State of the County Health Report (SOTCH), and the Community Health Improvement Plan (CHIP). Utilizing the Clear Impact Scorecard™, a versatile management and performance assessment tool, this document enables organizations to systematically gather, analyze, and disseminate pertinent data. This robust accountability instrument not only scales organizational effectiveness but also enhances program performance, while transparently communicating progress towards set objectives to stakeholders, funders, partners, and the wider public. Facilitating collaboration and data sharing among stakeholders, the Scorecard plays a pivotal role in fostering partnerships aimed at achieving collective goals. Moreover, through its web-based interface, the Scorecard streamlines community health improvement initiatives. The Iredell Health Department has embraced the Clear Impact Scorecard™ to facilitate the creation of electronic CHA, CHIP, and SOTCH Reports across communities in Iredell County.
