The Columbus County Health Department is excited to share the Healthy NC 2030 Scorecard for Columbus County. This Community Health Improvement Scorecard is an easy way to learn about some of the efforts currently underway in Columbus County to address three health priorities identified in the 2023 Community Health Assessment.

Columbus County is a rural county in southeastern North Carolina with an estimated population of approximately 50,000 residents. The Columbus County Health Department serves a predominantly rural and geographically dispersed population, with a significant proportion of residents experiencing economic hardship, limited access to healthcare, and chronic disease burden. Local economic conditions and social determinants of health contribute to health disparities that guide priority setting and performance improvement efforts reflected in this Scorecard.
