This scorecard represents the work-in-progress to improve the THRIVE infant vitality program at Canton City Public Health.

Vision: Canton City Public Health will be the agent of change to promote policy to ensure that all residents can actualize their greatest health potential, by addressing physical, mental, and environmental health.

Commitment to Racial Equity:
A shared commitment to racial equity centers the work in these important ways:
- shifting power intentionally to THRIVE stakeholders in decision-making and authority;
- assessing disaggregated data regularly by race and ethnicity for key health priorities;
- exploring what creates disproportionality through analysis of the factors in the system or society that lead to disparate
- outcomes for children based on race and ethnicity; and
- developing and implementing strategies to improve outcomes for BIPOC children and youth.
How the Scorecard Works:
The scorecard first describes the results that stakeholders seek to achieve, followed by indicator data that shows how the community is doing in reaching the result.
After that follow a set of programs (defined as strategies and actions) to achieve the desired results. A performance measure is attached to each program to help monitor the ongoing progress for it.
Results Based Accountability Framework:
Results-Based Accountability (RBA) uses a data-driven, decision-making process to help communities and organizations get beyond talking about problems to taking action to solve problems. RBA starts with ends and works backward, towards means. The “end” or difference you are trying to make looks slightly different if you are working on a broad community level or are focusing on your specific program or organization.
