The North Carolina State Health Improvement Plan (NC SHIP) Community Council brings communities and partners across the state together to work towards all people in North Carolina having equitable opportunities for health, education, and economic stability throughout the lifespan. Work group efforts advance the 21 health indicators identified in Healthy North Carolina 2030: A Path Toward Health (HNC 2030).
Using Results-Based Accountability, a data-driven framework, the 2024-2025 NC SHIP Community Council Scorecard monitors progress on state-level priorities aimed at improving quality of life in North Carolina.
Local health departments and their partners link local scorecards to the Healthy North Carolina 2030 Scorecard to reflect the collective impact occurring statewide on 21 population indicators.
