Conducted every four years, the Community Health Assessment (CHA) report captures a comprehensive picture of Guilford County’s health—through the lenses of environment, society, behavior, and lived experience—and identifies key areas for community health improvement planning. This assessment comes in the wake of the unprecedented COVID-19 pandemic that brought into focus persistent health and social inequities present in Guilford County.
This Healthy 2030 Scorecard for Guilford County (2023-2027) is a living document that uplifts the 2023-2024 CHA health priorities and the developing 2024 Community Health Improvement Plans (CHIPs). A CHIP is a multi-year plan that outlines collective efforts to tackle specific CHA health priorities.
This Scorecard is an easy way to learn about efforts currently underway in Guilford County to address 2023-2024 CHA health priorities:
- Firearm (Gun) Violence and Injury
- HIV and Other Sexually Transmitted Infections (STIs)
- Behavioral Health - Drug Overdose and Mental Health
- Healthy Eating and Active Living
- Maternal and Child Health and Infant Mortality
- Access to Health Care
For these priorities, this Scorecard spotlights:
- A Result Statement, a picture of where we would like to be,
- Important local Indicators or measures of how we are doing linked to Healthy NC2030 indicators,
- Select Programs, Activities, Coalitions, Strategies, and Clinical Care efforts, and
- Key Performance Measures that show how those programs are making an impact.
The Scorecard also includes the annual Guilford County State of the County Health reports (SOTCH), which are completed annually between assessments.
