HNC 2030 Scorecard: Swain County (2024-2026)

 

Swain County is committed to fostering healthier communities through informed action and collaboration. As part of the 2024 Community Health Assessment (CHA), three key health priorities were identified: Chronic Disease, Substance Use, and Mental Health. These priorities, developed through a Results-Based Accountability (RBA) approach, build upon the long-term goals outlined in previous CHAs and reflect the most pressing needs of the community.

In partnership with WNC Healthy Impact and other regional stakeholders, Swain County actively aligns resources and strategies to address these priorities. This collaborative effort aims to enhance physical and mental health, reduce disparities, and ensure access to essential services for all residents.

The Swain County Community Health Improvement Plan (CHIP) integrates these priorities into a focused, actionable framework. The CHIP Scorecard, serves as a tool to monitor and evaluate progress toward achieving the county’s health objectives.
