HNC2030 Scorecard: Polk County (2024-2026)

Polk County's Public Health and Community Partners employed an iterative and highly collaborative process for developing its 2024 Community Health Improvement Plan (CHIP), illustrated in this Scorecard format. We spent the first months of the 2024 cycle analyzing new and existing data; engaging in community conversations to establish priorities based on perceived relevance, impact, and feasibility of addressing concerns; translating perspectives into our 2024 Community Health Assessment (CHA) Report; and building a blueprint for community action that is grounded in evidence and results-based accountability.

Together with over 30 representatives of various community health and social support sectors, programs, and communities, the multidisciplinary Polk County team selected this cycle's three health priorities and goals:

• Mental Health: All Polk County community members attain greater mental and behavioral health through supportive community connections, increased awareness of services, and service accessibility. 
• Substance Misuse: All substance-  affected individuals and families fare better physically and emotionally against the impacts of substance use.
• Healthy Eating/Active Living (HEAL): All Polk County citizens live healthier, more active lifestyles through fostering community connection.  

Within each priority area, experts and community members alike continue to pool perspectives, efforts, and resources to develop and implement the Community Health Improvement Plan.

Our ultimate aim is to coordinate solutions across service delivery systems and disciplines, through multisectoral collaboration, to address our 2024 community health priorities in a targeted, but holistic way. In doing so, we hope to see concrete, measurable improvements in health across the Polk County community.
