A Healthier Wilson County Starts Here
Good health is the foundation for a strong and thriving community. In Wilson County, we are working together to make it a reality for everyone. The Healthy 2030 Scorecard is our way of tracking progress and sharing
the steps we are taking to improve the health and well-being of our neighbors.
The 2024 Community Health Needs Assessment (CHNA) identified the top health needs in our community and serves as the foundation for this plan. Through the Wilson Wellness Collaborative, local partners from healthcare, schools, businesses, faith-based organizations, and community groups are joining forces to address these needs and create lasting change.
The Healthy 2030 Scorecard is also our Community Health Improvement Plan (CHIP). This plan helps us stay accountable and meet the requirements for NC Local Health Department Accreditation. It is a living document
that grows and changes as we do. It highlights the strategies, programs, and results that are shaping a healthier Wilson County.
