The Community Health Assessment is an assessment by and for Carteret County residents to provide us with a true picture of the health and needs of Carteret County residents in order to equally and equitably guide decision-making, programs, and policies to improve health outcomes.
The Carteret County Health Department is excited to share the Healthy NC 2030 Scorecard for Carteret County. This Community Improvement Scorecard is an easy way to learn about some of the efforts currently underway in Carteret County to address the three health priorities identified in the 2024 Carteret County Community Health Assessment (CHA):
- Chronic Health Conditions
- Family Community and Social Support
- Mental Health
- Substance Misuse/Tobacco Use
This Scorecard also serves as Carteret County community health improvement plan (CHIP), fulfilling the NC Local Health Department Accreditation requirements that local health departments complete two CHIPs following the CHA submission and a State of the County's Health Report for Carteret on years when not completing the CHA.
For each priority, this Scorecard spotlights:
- A Result Statement, a picture of where we would like to be,
- Important local Indicators or measures of how we are doing linked to Healthy NC2030 indicators and
- Select Programs or activities and
- Key Performance Measures that show how those programs are making an impact.
