HNC 2030 Scorecard: Montgomery County (2024-2027)

The Montgomery County Department of Health is excited to share the Healthy NC 2030 Scorecard for Montgomery County. This Community Health Improvement Scorecard is an easy way to learn about some of the efforts currently underway in Montgomery County to address three healthy priorities identified in the 2024 Montgomery County Health Assessment. 

• All people in Montgomery County have equitable access to care that promotes their whole-person well-being through affordable services, appropriate transportation, and a variety of care options.
• All people in Montgomery County have equitable access to coordinated support networks related to substance use that promote connectedness, recovery and long-term wellness. 
• All people in Montgomery County have access to culturally appropriate resources that support whole person health and social well-being.
