HNC 2030 Scorecard: Transylvania County (2024-2027)

Transylvania Public Health is committed to protecting and improving the health of all people in Transylvania County. One way we do this is through the Community Health Assessment (CHA) process, conducted every 3 years in collaboration with community members, hospitals, and other partners like WNC Health Network. (Click here to learn more about the CHA process.)

The result is a Community Health Assessment document that describes current health indicators and status of the community, and what needs to change to reach a community's desired health-related results. It also lists priority health issues identified by community members as being most important to focus improvement efforts. The CHA is published as a PDF document and was submitted to the NC Division of Public Health in July 2025. (Click here to view the 2024 Community Health Assessment.)

The final phase of the CHA process is developing a Community Health Improvement Plan (CHIP) to identify measures related to these priority health issues and determine what actions we as a community will undertake to improve those outcomes. The CHIP is published electronically using this web-based Scorecard platform format and submitted to the NC Division of Public Health in September 2025.

In the years when we do not conduct a CHA, instead we produce a State of the County's Health (SOTCH) report, which details the progress made in the past year on implementing the CHIP action plans and improving priority health outcomes. These reports are also published electronically using the Scorecard platform and submitted to the NC Division of Public Health in March 2026 and March 2027.
