HNC 2030 Scorecard: Washington County (2024-2027)

The Martin-Tyrrell-Washington District Health Department is excited to share our Healthy NC 2030 Scorecard for Washington County. This scorecard supports the Community Health Improvement Plan (or CHIP), which guides the long-term, systematic efforts for addressing public health issues in Washington County based on the results from the Community Health Assessment (CHA). The scorecard outlines the efforts being made in Washington County to address three of the health priorities identified in the CHA:
