The Forsyth County Department of Public Health is pleased to present the Healthy NC 2030 Scorecard for Forsyth County (NC). This Community Health Improvement Scorecard provides an accessible way to learn about some of the initiatives currently underway to address the key health priorities identified in the 2025 Forsyth County Community Health Assessment (CHA):

This scorecard also serves as the Forsyth County Department of Public Health’s Community Health Improvement Plan (CHIP), fulfilling the NC Local Health Department Accreditation Requirement to complete two CHIPs following a CHA submission. A State of the County Health (SOTCH) report is submitted in the years when a CHA is not required. Forsyth County follows a four-year CHA cycle.
