A Community Health Assessment (CHA) is a comprehensive process conducted every four years by the Alexander County Health Department, as required by the North Carolina Department of Health and Human Services, to better understand the health needs of the community. It combines existing data with input gathered through surveys and focus groups to identify key health concerns, trends, and priorities.
Through collaboration with community partners, including the Healthy Alexandrians Task Force, advisory boards, and local leaders, the CHA helps guide decision-making, prioritize health issues, and inform strategies to improve health outcomes. This data-driven and community-informed approach ensures that efforts reflect the true needs of Alexander County residents and support long-term, meaningful improvements in community health.
Building on the findings of the CHA, the Community Health Improvement Plan (CHIP) serves as a strategic roadmap for addressing identified priorities. The CHIP outlines specific goals, strategies, and actions developed in partnership with community stakeholders to improve health outcomes. It provides a structured approach to implementing solutions, tracking progress, and ensuring accountability over time.
This Scorecard is being used as the CHIP to illustrate the priority health needs in Alexander County as identified through our 2025 Community Health Assessment process and the efforts to address them.
