What We Do
Sampson County Health Department's Diabetes Self-Mangement Education and Support (DSMES) is a program that focuses on healthy eating, physical activity and monitoring blood sugar as a way to improve health outcomes and quality of life for people with diabetes. DSMES consists of an initial assessment, a two-part individual or group class and a three-month follow-up assessment with a Registered Nurse/Diabetes Educator.
Who We Serve
DSMES serves Sampson County residents ages 18 and older that have been diagnosed with Type 1, Type 2, or Gestational diabetes.
How We Impact
By focusing on self-care behaviors, participants learn ways to manage and improve hemoglobin A1c, blood pressure, weight (BMI) and cholesterol. Participants also learn about medications, better nutrition, physical activity, and fewer or less severe diabetes-related complications to help them reach their self-management goals.
