The Yadkin County State of the County Health (SOTCH) report provides information for the health priorities identified in the Yadkin County Human Service Agency's 2023 Community Health Assessment (CHA). The report serves as an update of action taken to address these priorities and highlights the ongoing need for Yadkin County to collectively address social determinants of health through our Community Health Improvement Plans (CHIPS).
